Vitamin D3 is not biologically active and needs to be hydroxylated twice before it becomes biologically active. First, vitamin D3 is transported to the liver after binding to vitamin D-binding proteins in the blood. In the liver, it is transformed into 25-hydroxyvitamin D3 [25-(OH)-D3] catalyzed by 25-hydroxylase; after entering the circulation, it is also bound to vitamin D binding proteins and subsequently transported to the kidneys, where it is catalyzed by renal 1α-hydroxylase to produce the hormonally active metabolite 1,25-dihydroxyvitamin D3 [1,25-(OH)2-D3], which is the active vitamin D3, also known as osteotriol or calcitriol in Chemicalbook.

After entering the bloodstream, osteotriol also binds to vitamin D-binding proteins and reaches various tissues and organs, such as the small intestine, bones and kidneys, where it binds to the vitamin D receptors in the tissue cells and exerts its biological effects. Alfacalcitol [1α-(OH)-D3] is a product of the direct activation of vitamin D3 by 1α-hydroxylase in the kidney and is present in very small amounts in the body. Both osteotriol and alfacalcidol can be synthesised artificially. They can be used as medicines but are not usually supplemented as nutrients.
